Client participated in treatment to address symptoms associated with ADHD, including difficulty with attention, organization, task completion, impulsivity, emotional regulation, or follow through with daily responsibilities.

Treatment focused on psychoeducation, executive functioning strategies, coping skills, emotional regulation, routine development, and support with applying skills across home, school, work, or daily life.
Progress Made
Client demonstrated progress in identifying ADHD related challenges and using strategies to support organization, focus, planning, emotional regulation, and task completion.
Continued Care Recommendations
Client may continue to benefit from structured routines, external supports, coping skill practice, and follow through with recommended academic, occupational, medical, or community supports as appropriate.
Discharge Plan
Client is discharged at this time with recommendation to continue using learned skills and seek additional support if symptoms increase or functioning declines.
