Anxiety Discharge Summary Template
Use this basic template when documenting discharge for clients who received treatment related to anxiety symptoms, worry, avoidance, panic, or stress related impairment.
Clinical Pearl: Discharge can reinforce that anxiety progress often means improved coping and reduced avoidance, not the complete absence of anxiety.
Reason for Treatment
Client participated in treatment to address anxiety related symptoms, including excessive worry, avoidance, nervousness, panic symptoms, difficulty managing uncertainty, or impairment in daily functioning.
Course of Treatment
Treatment focused on psychoeducation, coping skills, grounding strategies, cognitive restructuring, gradual exposure or approach based strategies, emotional regulation, and reduction of avoidance patterns.
Progress Made
Client demonstrated progress in identifying anxiety triggers, using coping strategies, challenging anxious thoughts, increasing tolerance for discomfort, and improving functioning in identified areas.
Continued Care Recommendations
Client may continue to benefit from ongoing coping skill practice, gradual exposure to avoided situations, support system use, and continued monitoring of anxiety symptoms.
Discharge Plan
Client is discharged at this time with recommendation to continue using learned strategies and seek additional support if anxiety symptoms increase or interfere with functioning.
