Autism Discharge Summary Template
Use this basic template when documenting discharge for clients who received treatment related to autism related support needs, emotional regulation, social communication, routines, sensory needs, or functional skills.
Clinical Pearl: Discharge can emphasize supports, self understanding, and strategies that help the client continue functioning in ways that fit their needs.
Reason for Treatment
Client participated in treatment to address autism related support needs, including emotional regulation, social communication, flexibility, sensory stressors, routines, coping skills, or daily functioning.
Course of Treatment
Treatment focused on psychoeducation, emotional identification, coping strategies, communication support, routine development, sensory awareness, caregiver collaboration when appropriate, and strengthening adaptive functioning.
Progress Made
Client demonstrated progress in identifying needs, using coping or regulation strategies, improving communication, increasing self advocacy, or strengthening routines and supports.
Continued Care Recommendations
Client may continue to benefit from consistent routines, sensory supports, communication strategies, caregiver or school collaboration, and continued support with self advocacy and regulation.
Discharge Plan
Client is discharged at this time with recommendation to continue using identified supports and seek additional services if emotional, social, sensory, or functional needs increase.
