Depression Discharge Summary Template
Use this basic template when documenting discharge for clients who received treatment related to depressive symptoms, low motivation, mood concerns, withdrawal, or functional impairment.

Reason for Treatment
Client participated in treatment to address depressive symptoms, including low mood, reduced motivation, withdrawal, negative self perception, sleep or appetite changes, hopelessness, or decreased functioning.
Course of Treatment
Treatment focused on psychoeducation, behavioral activation, coping skills, emotional processing, cognitive restructuring, routine development, support system use, and strategies to improve daily functioning.
Progress Made
Client demonstrated progress in identifying mood patterns, using coping strategies, increasing engagement in routines or meaningful activities, and improving emotional awareness or functioning.
Continued Care Recommendations
Client may continue to benefit from consistent routines, continued coping skill use, behavioral activation, support system engagement, and monitoring for recurrence of depressive symptoms.
Discharge Plan
Client is discharged at this time with recommendation to continue using learned skills and seek additional support if mood symptoms return, safety concerns emerge, or functioning declines.
