Use this basic template when documenting discharge for clients who received treatment related to grief, bereavement, adjustment to loss, emotional processing, or re-engagement with life after loss.

Reason for Treatment
Client participated in treatment to address grief related distress, including sadness, longing, guilt, anger, numbness, difficulty adjusting to loss, reduced functioning, or challenges re-engaging with meaningful activities.
Course of Treatment
Treatment focused on psychoeducation, emotional processing, coping skills, meaning-making, continuing bonds, adjustment to life after loss, and gradual re-engagement with routines, relationships, and values.
Progress Made
Client demonstrated progress in identifying grief responses, tolerating difficult emotions, using coping strategies, honoring the significance of the loss, and taking steps toward continued life engagement.

Client may continue to benefit from support system use, grief informed coping strategies, meaningful remembrance, continued emotional processing, and monitoring for increased impairment or safety concerns.
