Use this basic template when documenting discharge for clients who received treatment related to obsessive thoughts, compulsive behaviors, reassurance seeking, avoidance, or distress tolerance.

Reason for Treatment
Client participated in treatment to address OCD related symptoms, including intrusive thoughts, compulsive behaviors, reassurance seeking, avoidance, distress intolerance, or impairment in daily functioning.
Course of Treatment
Treatment focused on psychoeducation, identifying obsessions and compulsions, reducing avoidance, building distress tolerance, practicing response prevention strategies when clinically appropriate, and supporting coping skills.
Progress Made
Client demonstrated progress in recognizing OCD patterns, increasing insight, reducing reliance on compulsive responses, tolerating uncertainty, and using coping strategies to manage distress.
Continued Care Recommendations
Client may continue to benefit from ongoing practice with response prevention strategies, reduction of reassurance seeking, support with uncertainty tolerance, and monitoring of symptom recurrence.
Discharge Plan
Client is discharged at this time with recommendation to continue using learned strategies and seek additional support if OCD symptoms increase or begin interfering with functioning.
