Goal Completion Language
Use this section for brief language describing progress toward treatment goals. Choose the phrase that best matches the client’s current progress and clinical presentation.
Goal Met
- Patient has demonstrated consistent progress and has met the identified goal.
- Client has achieved the treatment goal and demonstrates improved functioning in this area.
- Patient no longer requires active intervention for this specific goal at this time.
Goal Mostly Met
- Patient has made significant progress toward the goal, with some areas still requiring continued attention.
- Client demonstrates strong progress and continues to benefit from support with remaining needs.
- Patient is nearing completion of this goal and requires continued support to strengthen consistency.
Goal Partially Met
- Patient has demonstrated some progress toward the goal but continues to experience challenges in this area.
- Client has made partial progress and would benefit from continued skill building and support.
- Patient shows improvement but has not yet demonstrated consistent progress across settings or situations.
Goal Minimally Met
- Patient has demonstrated limited progress toward the goal despite continued therapeutic intervention.
- Client continues to experience barriers that have limited progress toward this goal.
- Patient may require continued support, modified interventions, or reassessment of treatment needs.
Goal Not Met
- Patient has not demonstrated meaningful progress toward the goal at this time.
- Client continues to experience symptoms or challenges that interfere with progress toward this goal.
- Patient may benefit from revisiting the goal, identifying barriers, and adjusting the treatment approach.
Goal Maintained
- Patient has maintained progress toward the goal and continues to demonstrate stability in this area.
- Client continues to use skills and supports to maintain progress and prevent regression.
- Patient demonstrates ongoing stability with no significant decline in this goal area.
Goal Discontinued
- Goal is no longer clinically indicated or relevant to the patient’s current needs.
- Goal has been discontinued due to changes in treatment focus, clinical need, or client priorities.
- Patient’s current presentation indicates that treatment should focus on other goals or areas of need.
