Insight
Insight may describe the client’s ability to recognize symptoms, triggers, needs, or patterns.
Common insight descriptors may include:
- Good
- Fair
- Limited
- Developing
- Variable
- Reduced
Judgment
Judgment may describe decision making, impulse control, safety awareness, and ability to consider consequences.
Common judgment descriptors may include:
- Good
- Fair
- Limited
- Impulsive
- Improving
- Impaired in specific situations
Example Documentation Language
- Client demonstrated fair insight into current symptoms and contributing stressors.
- Patient showed developing insight into behavior patterns and emotional triggers.
- Client demonstrated good judgment and was able to identify safe coping options.
- Patient demonstrated limited judgment related to impulsive decision making.
- Client showed improved insight into the connection between symptoms, choices, and functional impact.
