Reduce the Impact of Substance Use on Daily Life
- Client will identify at least 3 personal triggers and 3 consequences associated with substance use within the next 30 days.
- Patient will track substance-use patterns, cravings, triggers, and related functional effects at least 3 days per week for 4 consecutive weeks.
- Client will identify at least 2 harm reduction or recovery supporting changes and practice them in identified high-risk situations over the next 90 days.
- Patient will demonstrate improvement in substance-use-related functioning as evidenced by improved ASI or BAM scores from baseline to reassessment.
Build Healthier Ways to Cope Without Relying on Substances
- Client will identify at least 3 emotional, interpersonal, or environmental triggers that increase cravings or urges within the next 30 days.
- Patient will learn and practice at least 3 alternative coping strategies for managing cravings, stress, or emotional distress over the next 90 days.
- Client will use an identified coping strategy in at least 3 reported high-risk or emotionally activating situations per week for 4 consecutive weeks.
- Patient will report increased confidence in managing cravings without substance use from baseline to reassessment over the next 90 days.
Strengthen Motivation for Recovery and Meaningful Change
- Client will identify at least 3 personally meaningful reasons for change within the next 30 days.
- Patient will explore at least 3 perceived benefits and 3 costs associated with current substance use patterns during treatment.
- Client will identify and complete at least one realistic change oriented action each week for 4 consecutive weeks.
- Patient will demonstrate increased readiness for change as evidenced by improved SOCRATES or TEA scores from baseline to reassessment.
Maintain Recovery and Reduce Risk of Recurrence
- Client will develop a relapse prevention plan identifying at least 3 warning signs, 3 coping responses, and 2 recovery supports within the next 60 days.
- Patient will identify at least 3 high-risk situations and develop a specific response plan for each within the next 60 days.
- Client will contact an identified recovery support or use an established coping response during at least 2 reported high-risk situations over the next 90 days.
- Patient will review and update the relapse prevention plan at least once per month throughout treatment based on current needs and emerging risks.
Improve Functioning and Relationships Affected by Substance Use
- Client will identify at least one area of functioning affected by substance use, such as relationships, work, school, health, finances, or daily responsibilities, within the next 30 days.
- Patient will implement at least 2 specific behavioral changes intended to improve functioning in the selected area over the next 90 days.
- Client will practice at least 2 communication, boundary setting, accountability, or repair strategies in relevant interpersonal situations over the next 90 days.
- Patient will demonstrate improvement in the selected functional area as evidenced by client self report, clinical observation, and improved ASI or TEA scores from baseline to reassessment.
