Missed Appointment
Use these examples when documenting a missed appointment, no-show, or unsuccessful attempt to complete a scheduled session. Language should remain factual, neutral, and consistent with practice policy, payer requirements, and applicable clinical documentation standards.
General Missed Appointment Statements
- Patient did not attend the scheduled appointment. No clinical services were provided during this appointment time.
- Client was scheduled for session but did not present for the appointment. Therapist documented nonattendance according to practice policy.
- Patient missed the scheduled appointment and did not participate in session. No therapeutic intervention was provided.
- Client did not attend the scheduled session. Follow up will occur according to clinic policy and treatment needs.
- Patient was not present for the scheduled appointment time. Appointment was marked as missed/no-show.
No-Show With Outreach Attempt
- Patient did not attend the scheduled appointment. Therapist attempted outreach by phone and left a message when appropriate.
- Client missed the scheduled session. Therapist attempted to contact client regarding the missed appointment and encouraged rescheduling.
- Patient did not present for session. Therapist attempted outreach to assess interest in continuing services and to support rescheduling.
- Client was not present for the scheduled appointment. Therapist attempted follow-up contact and documented the missed appointment according to policy.
- Patient missed appointment. Outreach was attempted to address attendance, confirm continued interest in treatment, and review next steps.
Telehealth Missed Appointment
- Patient did not join the scheduled telehealth session. Therapist remained available during the appointment window according to practice policy.
- Client did not log in for the scheduled telehealth appointment. No clinical services were provided.
- Patient was scheduled for telehealth session but did not present in the virtual appointment room.
- Client did not attend the scheduled telehealth session. Therapist attempted contact when clinically appropriate and documented the missed appointment.
- Patient did not connect to the scheduled telehealth appointment. Follow-up will occur according to practice attendance policy.
Late Cancellation / Unable to Attend
- Client cancelled the scheduled appointment with insufficient notice according to practice policy. No clinical services were provided.
- Patient was unable to attend the scheduled session and appointment was not completed.
- Client notified therapist that they were unable to attend the scheduled appointment. Session was not held.
- Patient cancelled or missed the appointment and will be encouraged to reschedule when appropriate.
- Client was unable to participate in the scheduled appointment. Attendance expectations and rescheduling needs may be reviewed at next contact.
Repeated Missed Appointments
- Patient has missed multiple scheduled appointments, which may impact treatment consistency and progress toward goals.
- Client’s repeated missed appointments have limited continuity of care and reduced opportunities for intervention, monitoring, and skill development.
- Patient’s attendance pattern will be reviewed to determine barriers to engagement and continued treatment needs.
- Client may benefit from discussion of scheduling barriers, motivation for treatment, and strategies to improve consistency.
- Continued missed appointments may require review of attendance policy, treatment readiness, or appropriate level of care.
Clinical Risk Considerations
- Due to missed appointment and known clinical risk factors, therapist attempted follow-up contact and reviewed appropriate next steps according to clinical judgment and policy.
- Patient did not attend the scheduled appointment. Given history of safety concerns, therapist considered risk status, available supports, and need for outreach.
- Client missed session. Therapist will continue to monitor attendance and risk concerns during future contact.
- Patient’s missed appointment limited therapist’s ability to assess current symptoms, functioning, safety, and treatment progress.
- If client cannot be reached and risk concerns are present, therapist will follow applicable safety, supervision, and practice protocols.
Documentation Language for Treatment Impact
- Missed appointment limited opportunity to review symptoms, assess safety, and support progress toward treatment goals.
- Nonattendance impacted continuity of care and delayed planned therapeutic interventions.
- Missed session prevented review of homework, coping skill use, treatment progress, and current clinical needs.
- Appointment nonattendance limited therapist’s ability to assess current functioning and update treatment recommendations.
- Continued inconsistent attendance may interfere with treatment progress and should be addressed collaboratively.
Follow-Up and Rescheduling Language
- Therapist will encourage client to reschedule and will review attendance expectations at next contact.
- Patient will be offered opportunity to reschedule according to availability and practice policy.
- Therapist will follow up regarding continued interest in services and any barriers to attendance.
- Attendance expectations, scheduling barriers, and treatment engagement will be reviewed during the next completed session.
- Client will be reminded of cancellation and attendance policies as clinically appropriate.
