Use these examples when documenting telehealth sessions, virtual session logistics, technology issues, privacy considerations, location verification, engagement concerns, caregiver involvement, or clinical adjustments made in a remote setting. Language should remain objective, clinically grounded, and specific while documenting how care was provided and how treatment needs were addressed.
Clinical Pearl: Telehealth can still provide meaningful therapeutic connection, structure, and progress. Technology barriers, distractions, or environmental limitations are common and can be addressed clinically through flexibility, clear expectations, rapport building, and collaborative problem solving.
General Telehealth Statements
- Session was conducted via telehealth, and client participated from a private or clinically appropriate location.
- Patient engaged in virtual session and discussed current symptoms, stressors, coping strategies, and treatment related needs.
- Telehealth format supported continued access to care and allowed client to participate in treatment despite scheduling, transportation, health, or location barriers.
