Thought Content
Thought content may include clinically relevant themes, concerns, beliefs, worries, or risk factors discussed during session.
Common content descriptors may include:

- Worry or rumination
- Negative self talk
- Hopelessness
- Trauma related content
- Substance use related concerns
- Safety related concerns
Risk Factors
Risk factors may include:
- None reported
- Suicidal ideation
- Homicidal ideation
- Self harm
- Other safety concerns
Example Documentation Language
