Goal 1: Reduce the impact of substance use on daily life
Objective:
Over the next 90 days, patient will identify at least 3 personal triggers and 3 consequences associated with substance use and review them during treatment, as documented in session notes.
Measurement Tools (when appropriate):

- Addiction Severity Index (ASI)
- Brief Addiction Monitor (BAM)
Goal 2: Build healthier ways to cope without relying on substances
Objective:
Over the next 90 days, patient will identify and practice at least 3 alternative coping strategies for managing cravings or emotional distress and report use of at least one strategy between sessions when clinically appropriate.

Goal 3: Strengthen motivation for recovery and meaningful change
Objective:
Over the next 90 days, patient will identify at least 3 personal reasons for change, discuss ambivalence, and identify one realistic next step aligned with their current readiness for change.

Goal 4: Maintain recovery and reduce risk of recurrence
Objective:
Over the next 90 days, patient will develop a written or verbal relapse-prevention plan identifying at least 3 warning signs, 3 coping responses, and 2 recovery supports.

Goal 5: Improve functioning and relationships affected by substance use
Objective:
Over the next 90 days, patient will identify one functional area affected by substance use and implement at least 2 specific behavioral changes intended to improve functioning in that area.
