Goal 1: Reduce OCD Interference and Increase Daily Functioning
Objective:
Client will identify and track major OCD-related compulsions, avoidance patterns, and functional interference and use response-prevention strategies during treatment over the next 90 days.

- Compulsion frequency or duration tracking
- Avoidance tracking

Goal 2: Increase Ability to Tolerate Uncertainty and Distress
Objective:
Client will practice approaching selected uncertainty or obsessional triggers while reducing reliance on compulsive responses and monitoring distress and willingness over the next 90 days.
Measurement Tools (when appropriate):
- SUDS
- ERP practice tracking
- Client self report
- Clinical observation
Goal 3: Reduce Compulsive and Reassurance-Seeking Responses
Objective:
Client will identify behavioral and mental compulsions, including reassurance seeking or rumination when present, and practice delaying, reducing, or preventing selected compulsive responses over the next 90 days.

- Compulsion tracking
- Reassurance-seeking tracking
- Time spent ritualizing

Goal 4: Increase Values-Consistent Behavior
Objective:
Client will identify meaningful activities or responsibilities limited by OCD and take gradual, treatment-consistent steps toward increased participation while reducing OCD-driven avoidance over the next 90 days.

Goal 5: Build Confidence in Responding to OCD Without Seeking Certainty
Objective:
Client will practice responding to selected obsessional doubts without repeated reassurance, checking, or mental review and will report increased ability to leave appropriate questions unanswered over the next 90 days.

- Reassurance frequency tracking
- Checking/reviewing tracking
- Client self report
- Clinical observation
Clinical Note
Objectives should measure change in the OCD cycle, not simply the presence or absence of intrusive thoughts. A client can continue to experience intrusive thoughts while making substantial clinical progress if compulsions and avoidance decrease and functioning improves.
Standardized measures can support treatment planning and outcome monitoring but do not independently establish an OCD diagnosis.
OCD Objectives
- Client will reduce OCD symptom severity over the next 90 days, as measured by the Y-BOCS or OCI-R.
- Patient will practice response-prevention strategies during identified OCD triggers at least 3 times per week over the next 90 days.
- Client will increase participation in at least 2 meaningful daily activities per week over the next 90 days, despite OCD-related distress.
